Under the terms and conditions of Field Haus USA, being aware of my your health and physical condition, and having knowledge that your participation in any exercise program may be injurious to your health, You are voluntarily participating in a physical activity and nutrition program. Having such knowledge, you hereby acknowledge and release any representatives, agents, and successors from liability for accidental injury or illness, which you may incur as a result of participating in the said physical activity and recommended nutrition program. You hereby assume all risks connected there with and consent to participate in said program. You agree to disclose any physical limitations, disabilities, ailments, or impairments, which may affect your ability to participate in said fitness program. You choose to participate of your own free will. In consideration of the permission to participate extended to you and for the services furnished to you do hereby for yourself, your heirs, spouse, children, unborn children, personal representatives, and agents release and forever discharge any and all claims, demands, actions or law suits on account of my injury or death that might occur as a result of negligence on the part of yourself or other persons affiliated or not affiliated with this contract. By participating you have agreed to the consent and you affirm that you have read and understand all of yours rights as outlined in this agreement.
